Applicants must submit a log extract certified correct and signed by two other amateurs,
under the following headings

Full Name, Address, Signature and Callsign of applicant are required

Fee (to accompany application): 3 Australian dollars (or equivalent) or 4 IRCs
Additional sticker can be applied for at a later date costing $1 Australian  per application
